Scuba Forms

INSTRUCTIONS
If you are applying to take scuba diving, you need to fill in the following forms as per the instruction and fax them to Wayne Smith.

1)The Safe Diving Form

2) The Medical Form

3) The Liability Form

4) The Adventure Diver Form (This form is not necessary for those applying to the Selection Camp)
